Pokemon Yellow, when I first saw Pikachu. My mind flashed back to the show, which I watched frequently.
Pikachu: "PIKACHU!"
*screams*

The Suffering.
Just about all of it.
Or, you know, F.E.A.R.

OBLIVION The Adoring Fan
You're walking across a quiet hallway in some Ayleid ruin, right? You expect some monsters or booby traps. You enter a big room and you get the feeling that something may be behind you when...
"BY AZURA! BY AZURA! BY AZURA!" The camera zooms in on a Wood Elf's face suddenly.
That can be the most scariest thing ever in a videogame sometimes.

I was playing Final Fantasy X-2 and I was at the final boss on the road to the Farplane. Well, the boss, Anima, used his special technique on me. At first I was all, "Please don't kill me!!" as he dragged me to the underworld or whatever to do his technique. Anima is a she, Chibi. But that's okay, everyone makes mistakes.

Yeah, that's my favorite part about Anima. That and the backstory her Fayth gives you.

Anima scared me at first, but when I found out that she was Serymour's mother, I took it all back and thought of her like a mother: Always taking care of her child. (in this case, her Summoner, Yuna)

Her Overdrive got an upgrade in the International version: 16 stinkin' hits instead of 1. Ahem, anyways... After you pick up the Door Knob in RE: CV and when the Hunters come at you. Jeez.... v.v

In Ocarina of Time, when I was in the Water Temple, and in the illusion room (the room where you fight Dark Link). Well, I go to the end of the room, go back to the middle, and--bang!--Dark Link's standing there! That made me jump. Oh yeah. That freaked me out. But when I use to play Ocarina of Time when I was younger (12 or so) I was always scared of the wall masters in the Forest and Shadow Temples. I always got my brother to beat them for me. -_-

^Those Wall Masters were a pain and they scared me too. I'm glad they were in the Forest Temple, gets them out of the way.
In Resident Evil for the Gamecube, there were those Crimson Heads that were added to the game. Just reading about them scared me, so I had to use the umlimited grenade flame rounds throughout the whole game b/c I knew I'd panic and accidently let them kill me somehow. Those suckers are mad fast.
