Gai probably started it without Kakashi realizing. Like, he saw Kakashi in action and just goes: That man is now my rival! MWAHAHA! And Kakashi had no clue until Gai started making him compete. At least, that's my theory.

The idea I've had (well for a fic since it really has no proof/evidence of this occuring, story is open to writter who would like to write it) is that Gai was assigned to Kakashi's team when Obito was killed at the end of Kakashi Gaiden. Gai developes a huge crush on Rin and tries to impress her. As it turns out Rin has a huge crush on Kakashi and this causes Gai to try and defeat him in order to impress her. Gai constantly loses his fight with Kakashi which results Gai to constantly train in order to defeat him.
First he losses in simple matches with Kakashi and eventually gets better to the point where Kakashi needs to use his sharrigan to beat him. Gai focuses on Taijutsu/Chakara Gates because he learns that whatever new ninjutsu move he developes to fight against Kakashi will just be copied and used against him.
Later some rouge ninja kills Rin and this causes the village to hunt down and capture him. Rin's killer is imprisoned and is sentanced to life in jail. Kakashi viewed that punishment not good enough, and is determined to break into prison so he can kill Rin's killer. Just when Kakashi was about to break in he is stopped by Gai. Gai informs Kakashi that the last thing Rin would want is Kakashi becoming a criminal to avenge her. Kakashi refuses to listen and forces Gai to stop him using force.
Kakashi brushes Gai threats since he knows that he is no match against him and his sharrigan eye (that and their fighting record is 30 to 0). In an amazing battle Gai defeats Kakashi for the first time with his newly developed anti-sharrigan fighting style. Only through defeat does Kakashi realize that Gai was right and gives up on his quest for revenge. Afterwards Kakashi begins to respect Gai (eventhough he would never admit it) and their playful rivalry begins.

I thought it was made fairly clear that Gai's rivalry with Kakashi is much like how Lee and Neji are/were as kids. Gai has more or less nothing but Taijutsu to work with, much like Lee.
The way Neji criticized Lee for thinking he could be a great ninja on taijutsu alone infuriates Lee to a point that he forces himself to train harder. I don't think Kakashi would be the sort of jerk to rub his abilities in a comrades face.. but both Kakashi and Gai are from the same generation of shinobi. I bet Gai saw Kakashi as being one of the strongest ninja of his era and made up these silly competitions to motivate himself.

I think one common misconception is that people think Gai can only perform Taijutsu. In reality he is actually capable of using ninjutsu but for some reason chooses not too. It is assumed that he is able to summon Ninkine (the turtle) and in the Invasion of the Leaf Village was able to dispel genjutsu. Also in the Naruto handbook it states that he scores 3 out of 5 in ninjustsu and uses chakara to enhance his Taijustu moves.
My theory on why he doesn't perform ninjutsu was because of his rivalry with Kakashi. He was decent at ninjutsu but Kakashi was always better than him. There was no point of him training in ninjustsu since any move he developed or learned could be copied by Kakashi's sharrigan eye and used against him. Instead of developing ninjustu that would be used against him in a fight, he decided to focus more on Taijustsu something Kakashi could copy but not perform due to its physical stress on the untrained body.
